C&M Heart Failure Pathway App

The Heart Failure app has been fully endorsed by C&M Cardiac Board and is an integrated end-to-end pathway, from primary care through to tertiary care, supporting improved access and outcomes for the management of patients with Heart Failure.The approach to pathway development has been multidisciplinary involving; patients, GPs, Cardiologists, rehab teams, Heart Failure specialist nurses, pharmacists and other healthcare specialists to support integration across organisational boundaries.The application allows access to the Heart Failure pathway to a national audience with the aim of improving prescribing practices in primary, secondary and tertiary care.Key features include:Interactive diagnosis and treatment diagnosisFull guideline text with bookmark and notes functionSharing content with colleaguesAccess to other useful resourcesDisclaimerThe app is intended to assist healthcare professionals and allied healthcare professionals as an educational tool to provide information that may support them in providing care to patients.
